He'll be talking to himself. but with this ending we are assuming he is chosing to stay in his delusion, to stay in the foggy silent hill. With Maria being 'alive' in his mind, he isnt going back to the real world.

Yes...I did love the nurses. The first movement scene made me chuckle, but after that I was genuinely a bit tense (not much scares me in movies)
