I think the overall theme of the otherworld is the same for all of them, but some certain elements might differ based on each character's psyche.
For example Heather would see elements related to her pregnancy, while Douglas 'might' see ones regarding his son's death.

While the worlds in SH4 were pretty much scary in context, I was actually more frightened by Henry's own apartment. (where you saved your game) Both before & after the hauntings started. Don't know if it was the 1st person camera view or the spooky atmosphere, it'd just scare the hell out of me....

James finds the flashlight on a mannequin wearing Mary's clothes, then the batteries there, & the hand reaching out of the door in Alternate hospital; I think they're somehow related.

Have you guys considered the short film "Fukuro" in Lost memories:Art & music of SH dvd? That's somehow sexual imo, & the fact that PH is the central figure in it...Well, I think 'natural urges' in the game means somewhere between sexuality & violence.
