I think there is only two points in the game where he comes out of nowhere. The rest of the time there will be a cutscene showing you where it is.
I really like how Pyramid Head is too strong for you, just because typical games you can just kill the enemy, even if it takes a long time, but just the fact that Pyramid Head is so much stronger makes the game so much more scarier, how the other monsters are easily below him. So neat
