If you mean how she died before the game took place, apparently someone (Probably Kaufman) murdered her. If you mean the scene in the game where she's bleeding, it's because she's turning into a demon. She was basically a really elaborate monster created by Alessa, similar to Maria from Silent Hill 2.

'Monster'? I don't believe so, because she proves to be more of an aid to Harry than anything.
I can't say I know much on Lisa, but I believe there are already a few threads on her if you want to get into that.
